Project Six for Adults provides a welcoming, home-like environment for adults with developmental disabilities. Our mission is to promote independence, self-determination, and community integration through individualized care and active skill-building. Each resident is supported by a dedicated team committed to helping them live a fulfilling life.
Residential Life
Residents at Project Six live in cozy, family-style homes where structure and support go hand-in-hand with freedom of choice. Daily routines are designed to turn everyday moments into opportunities for learning. This includes preparing meals, managing personal care, or practicing social skills. Community outings, such as trips to the beach, museums, and local events, encourage connection and confidence. With a staff-to-resident ratio of 1:3, every individual receives personalized attention in an inclusive setting.
Services Provided
Our program offers a full spectrum of habilitative services tailored to each resident’s goals. These include assistance with independent living skills, medication management, and vocational training, as well as access to medical and behavioral specialists. Residents benefit from individualized service plans developed by a multidisciplinary team, regular progress reviews, and active treatment strategies that promote growth. Recreational therapy, social activities, and community engagement are integral parts of the program, ensuring a holistic approach to well-being.
Project Six FAQs
Project Six’s Group Homes are designed to enable adults with developmental disabilities to function successfully in the mainstream of the community by maximizing self-determination, self-care, direction, and freedom of choice. This program facilitates the development of those skills necessary for each consumer to function more independently in a socially appropriate manner. Inherent to the philosophy of this program is the commitment to assist each consumer in the development of his/her potential for independence.
Project Six consists of four homes in residential neighborhoods in North Hills, CA.
Our homes are continuous care homes that are staffed 24 hours per day with Direct Care Support Staff and a Registered Nurse 40 hours per week. Project Six DD-Homes function continuously throughout the year.
In order to be considered for admission to Project Six, the individual must have been assessed by an interdisciplinary team and recommended for ICF-DD/H services. Project Six will only admit those consumers for whom the facility can meet their individual needs. Project Six will admit individuals, age 18 to 70 years, with a documented developmental disability who have been cleared from requiring skilled-nursing care by a physician.
Project Six is a licensed and Medi-Cal certified facility. We are licensed through the California Department of Public Health (CDPH). The Department of Developmental Services and Regional Centers are responsible for placement and quality assurance. Project Six is a Medi-Cal funded facility minus the consumer’s share of cost. Private pay is also an option. The daily rate for private pay for the services offered by Project Six is $353.60. Private pay participants are still required to obtain a referral from their registered Regional Center.
